Primary Responsibility / Summary

Responsible for outreach, assessment, development of individual employment plans and the removal of varying barriers to employment. Also responsible for working with Job Placement Specialists to establish a pipeline of candidates to fulfill hiring work orders.

Essential Duties & Responsibilities

  • Assists clients with gaining independence through life skills (financial management, employment, education, accessing social services, legal issues, housing, healthcare, personal hygiene and coping) through connection with partner agencies.
  • Works with local employers to develop and identify appropriate jobs for work experience opportunities.
  • Provides classroom instruction to groups and individuals on job seeking skills and techniques, motivation, resume preparation, application completion, job retention and Internet use for job search.
  • Provides participants with work skills assessments that will enhance their ability to find appropriate employment.
  • Works with local employers and participants in developing and identifying appropriate jobs and helping participants gain meaningful employment. Assists in reasonable accommodation process with prospective employers.
  • Maintains detailed electronic and hard copy case records according to funder and/or Company requirements. Completes all documentation in an accurate and timely manner.
  • Achieves enrollment and placement goals, and conducts timely follow‐up with all participants on caseload.
  • Meets with quality assurance team to assist with implementation strategies for continuous quality improvement.
  • Develops job leads and job orders to meet the needs of job seekers and employers. Interacts with job seekers and Career Service Specialists to ensure job leads match job seekers interests and skills
  • Assesses participants, creates individualized plans, and conducts job readiness workshops that prepare job seekers for real-life business needs and expectations.
  • Maintains detailed case records according to funder and/or company requirements. Completes documentation in an accurate and timely manner.  Conducts real time data entry inputting necessary information into automated case management systems, maintains electronic and hard copy confidential case files, documenting participant progress, prepares progress reports, case notes, and other documentation as required by program/funder. Providing data entry and documentation, to maintain schedules and timesheets.

Additional Duties & Responsibilities

  • This position may require extensive travel (>50% of the time.) A valid CA driver's license and state‐required auto insurance required. Driving record must be acceptable by the organization’s insurance vendor.

 Education & Experience

    • Minimum of 2 years of Case Management, Intake Specialist, or Social Services Work Experience.
    • Bachelor's degree from four‐year college or university in Rehabilitation Counseling or a related field preferred; High school diploma or general education degree (GED) required.
